Interim Healthcare of Delaware is a Medicare Certified Home Health Agency that provides both skilled care and personal care and support. We staff caregivers with clients in their residences, including group homes, as well as schools.

 

In additional to traditional home care, Interim Healthcare of Delaware specializes in both Consultative Nursing (Healthcare Management) and Newborn Pediatric Services. 

 

We are the leading state provider of Consultative Nursing (Healthcare Management) for individuals who have developmental disabilities. Our nurses (Care Managers) check on your loved ones to make sure the care they receive matches the care prescribed by their doctors.

 

We are one of the largest providers of Newborn Pediatric Services in the state. We perform physical overview and weight checks for at-risk newborns such as those with neonatal abstinence syndrome, born premature, and those exposed to social/economic risk.

Interim Healthcare accepts a variety of payment sources and insurances such as Medicare, Medicaid, Long Term Care Insurances, VA Programs, some Commercial Insurances and Private Pay options.

Caregivers

 

Licensed Practical Nurse (LPN)

Licensed Practical Nurse (LPN) participates in ongoing interdisciplinary assessment of the client while following the plan of care outlined by the physician.  Monitoring basic patient health such as vital signs and overall condition. Changing dressings or inserting catheters. Taking patient histories and maintaining documentation. Assisting with tests or procedures. Providing personal care, such as helping with bathing and toileting. Consulting with RNs on care plans. Effectively communicates with other members of the interdisciplinary healthcare team providing care to promote coordination of patient care and planning for discharge. Communicates with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care.

 

Occupational Therapist (OT)

Provide services for the purpose of treating impairments, functional limitations, disabilities or changes in physical function and health status for patients normally performed in their home, schools, or occupational settings

Registered Nurse (RN)

The Registered Nurse (RN) provides services and/or treatments requiring substantial and specialized nursing skill and provides effective and efficient patient care.  Participates in ongoing interdisciplinary assessment of the patient. Completes initial and ongoing comprehensive assessments of the patient’s needs at appropriate time points as assigned. Participates in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s). Contributes to the patient’s plan of care/service plan. Participates with the patient in clarifying mutually agreed upon goals and incorporates these goals into the plan of care/service plan. Provides services that are ordered by the physician as indicated in the plan of care. Counsels and educates the patient and family in meeting healthcare related needs.

 

Certified Nursing Assistant (CNA)

Provide all activities of daily living: bathing, grooming, dressing, ambulation, and transfers.  These caregivers also assist with meals,  light housekeeping duties, and safety.

Physical Therapist (PT)

Provide services for the purpose of treating impairments, functional limitations, disabilities or changes in physical function and health status for patients normally performed in their home, schools, or occupational settings

 

Home Health Aide (HHA)

Provide all activities of daily living: bathing, grooming, dressing, ambulation, and transfers.  These caregivers also assist with meals,  light housekeeping duties, and safety.
